Created a Talent and Interest Survey Form for my wards YSA to use

The survey is a form that was created by The Church of Jesus Christ of Latter-Day Saints in 6/98 form 33812. We thought it would be useful for our committee chairpersons to use in plans for activities to meet the needs and interest of their group of young adults.

Its was created by using Google Docs. It was great to use because the survey will be collected into a excel spreadsheet which can be download. I will take the spreadsheet and append it into a FoxPro Database and report the results that I will publish for all of the 20 or more people planning activities for the young single adults.

The survey can be email to a list of people which make it nice so data entry is easy for responder and paper doesn't have to be used.

One thing that was added to the form was an email address and cell phone and if texting is used.

Maybe the next survey that we will do is the emergency resources that each ward should have for leaders.
